Part-time children will be assigned the same day/days each week.
Half Days are 5 hours or Less each day. Time over 5 hours is subject to the same charges as late pick-up. PAYMENT IS DUE FOR THESE DAYS WHETHER THEY ARE HERE OR NOT. Drop-in (only if space is available on a daily basis) is provided by our center.
This agency has a 2 week trial period for any enrolled child. This agency reserves the right to request the withdrawal of a child during or after the trial period if one or more of the following conditions exists: (1) the child is not participating in or not benefiting the program: (2) there are no reasonable accommodations this agency can make to provide adequate or safe care for this child; (3) there are no reasonable accommodations this agency can make to provide adequate or safe care to other enrolled children due to the care needs of this child.
There are three forms to be completed when a child becomes enrolled in the
center. These are the Child’s Application, Health Record, and the Child’s Health History Checklist. Parents must complete the application form and fill in each line as specifically as possible. The Health record must be completed by a health care provider. The health record must be updated each year or when the child receives additional shots. Obtain a copy of the form from the director when you are taking your child to the doctor. Annual health examinations are required and are the responsibility of the parent.
